Diagnosis

Form and email delivery checklist

Start by proving whether the form stored the submission. That single fact splits the job in half.

Print this page to keep a paper copy beside the work.

  1. 1 Submit the form yourself and see whether a row appears in the CMS, database, or form plugin inbox.
  2. 2 If the row is there and mail is not, the form works. The mail account, SMTP, or DNS authentication does not.
  3. 3 Confirm the SMTP user still exists. Deleted mail-service accounts are a common silent failure.
  4. 4 Check authentication and any recipient allowlist on staging copies as well as production.
  5. 5 Open the notification template and confirm it prints the submitted field variables, not a leftover placeholder.
  6. 6 If only some languages fail, look for overlapping captcha or challenge plugins.
  7. 7 Purge page cache after a form change. Cached HTML will keep serving the broken form.
  8. 8 Send a test that has to leave the server — not only an on-site “success” banner.
  9. 9 Record SPF, DKIM, DMARC, and the sending hostname so the next incident starts with facts.

A green “thank you” page is not proof of delivery.

Access usually needed

Website admin, hosting or SMTP provider access, DNS control, and the destination inbox are usually enough. If a CRM or automation receives leads, access to that tool helps confirm the full delivery path.

The form shows a thank-you page but we get no email. Is the form broken?

Overlapping captcha or challenge plugins are a common cause, and page cache will keep serving the broken markup after you “fix” it. We test each language and purge cache.
